The purpose of this section of our website is to inform physicians and their staff, in the Central Florida area, about the services available for patients with permanent vision loss and to help facilitate referrals to Lighthouse Central Florida's vision rehabilitation services.


As people live longer with eye conditions such as diabetes, macular degeneration, glaucoma, and cataracts, the number of patients with partial eye sight is increasing dramatically. While strides in medical technology have helped patients maintain a significant amount of their vision, many patients still have problems with daily visual function; this is where Lighthouse Central Florida can help. When vision cannot be fully restored through medicine, surgery, regular glasses or contact lenses, Lighthouse services can help.

Patients may still encounter challenges in daily life, such as:

• Reading and writing, even with glasses
• Identifying medications
• Moving around safely
• Getting or keeping a job
• Cooking safely
• Feelings of sadness and/or anxiety about their vision

Lighthouse Central Florida services can help your patients manage daily living activities and remain safe and as independent as possible. They are not just for the "active" individual. Many individuals who received services from Lighthouse Central Florida were "sedentary" before their experience at Lighthouse and became active once they learned how they can live with their vision loss and interact with their environment and community.

Lighthouse Central Florida would like to reach out to you as a physician to help facilitate the care for your patients. Our services compliment the medical and surgical treatment of eye conditions and ongoing care provided by eye doctors. Through partnerships with medical professionals, we can add to the continuum of care for patients with vision impairment.


Please use the Referral to Lighthouse Central Florida for Vision Rehabilitation form to connect your patients with our services. You may fax the form to Vanessa Daniels at 407-898-0236. Upon receipt we will contact the patient to arrange for the services that meet the individual's needs. 

PLEASE NOTE: This form is to only be used to refer patients in Orange, Seminole and Osceola counties.
